Penalties shall be actual playing time and shall be divided into the following classes:
(1) Minor Penalties
(2) Bench Minor Penalties
(3) Major Penalties
(4) Misconduct Penalties
(5) Match Penalties
(6) Penalty Shot
When penalties are imposed after the conclusion of any game and before the players have left the ice, such penalties shall be reported to the proper authorities by the Referee on the Official Game Sheet.
In cases when any rule states that the Manager or Coach shall designate a player to serve any penalty and if the Manager or Coach refuses to name a player, the Referee shall have the authority to name any player of the offending team that he desires to serve the penalty.

(a) For the first "MAJOR PENALTY" in any one game, except to the goalkeeper, the offender shall be ruled off the ice for five minutes, during which time no substitute shall be permitted .
(b) For the second Major Penalty in the same game to the same player or a Major Penalty for cross-checking, buttending, high-sticking, kneeing, elbowing, roughing, slashing, spearing, boarding and checking from behind, the player shall be ruled off the ice for the balance of the playing time, but a substitute shall be permitted after five minutes (Major Penalty plus Game Misconduct Penalty, see rules 604, 606, 607, 609, 612, 615, 630, 631).
(c) When Coincident Major and/ or Match Penalties or coincident penalties of equal duration including a Major or a Match are imposed against players of both teams during the same stoppage of play, immediate substitutions shall be made for those penalties, and such penalties shall not be taken into account for the purpose of the delayed penalty. In the case where the penalized players remain in the game, they shall take their place on the penalty bench and shall not leave until the first stoppage of play following the expire of their respective penalties. When the Coincident Major/ Match Penalty rule is applied and there is a differential in the total time penalties, those penalties causing the differential shall be served first in the normal manner and shall be taken in account for the purpose of applying Rule 402 (c) Minor Penalties and Rule 408 Delayed Penalties. Any time difference or differential in the total time penalties may be served by any non penalized player.

(a) For a "MINOR PENALTY" any player other than a goalkeeper shall be ruled off the ice for two minutes during which time no substitute shall be permitted.
(b) A "BENCH MINOR" penalty involves the removal from the ice of one player of the team against which the penalty is imposed for a period of two minutes. Any player, except the goalkeeper of the team, may be designated to serve the penalty by the Manager or the Coach, through the Captain, and such player shall take his place on the penalty bench promptly and serve the penalty as if it was a Minor Penalty imposed on him.
(c) If, while a team is "SHORT-HANDED" by one or more Minor or Bench Minor Penalties, the opposing team scores a goal, the first of such penalties assessed shall automatically terminate.
Note 1: "Short-handed" means that the team must be below the numerical strength of its opponents on the ice at the time the goal is scored. Thus, an equal number of Minor or Major Penalties to an equal number of players of both teams does not cause either team to be "short-handed"
This rule shall also apply when a goal is scored on a Penalty Shot.
When the Minor Penalties of two players of the same team terminate at the same time, the Captain of that team shall designate to the Referee which of such players will return to the ice first and the Referee will instruct the Official Scorer accordingly When a player receives a Major Penalty and a Minor Penalty at the same time, the Major Penalty shall be served first by the penalized player:
Note 2: This applies where BOTH penalties are imposed on the SAME player (see also Note to Rule 408, Delayed Penalties).
d) When ONE Minor Penalty is assessed to ONE player on EACH team at the same stoppage of play, these penalties will be served without substitution provided both teams are at full strength on the ice.
Unless paragraph one of this rule section is applicable, when Coincident Minor Penalties or Coincident Minor Penalties of equal duration are imposed against players of both teams, the penalized players shall all take their places on the penalty benches and such penalized players shall not leave the penalty bench until the first stoppage of play following the expire of their respective penalties. Immediate substitution shall be made for an equal number of Minor Penalties or Coincident Minor Penalties of equal duration to each team so penalized and the penalties of the players for which substitutions have been made shall not be taken into account for the purpose of the Delayed Penalty Rule (408).

(a) A "MISCONDUCT PENALTY" to any player, except the goalkeepers, involves removal from the game for a period of ten minutes. A substitute player is permitted to replace immediately a player serving a Misconduct Penalty. A player whose Misconduct Penalty has expired shall remain in the penalty bench until the next stoppage of play.
When a player receives a Minor or Major Penalty and a Misconduct Penalty at the same time, the penalized team shall immediately put a substitute player on the penalty bench and he shall serve the Minor or Major Penalty without change.
The second Misconduct Penalty to the same player in the same game shall automatically become a Game Misconduct Penalty.
(b) A "GAME misconduct" Penalty involves removal of a player or team official for the balance of the game and the offender shall be ordered to the dressing room for the remainder of the game, but a substitute shall be permitted immediately for the player.
Note 1: A "Game Misconduct" Penalty does not include automatic suspension, except for that game. The proper authority shall have the power to increase the suspension period.
Note 2: In championship and tournament games any player shall be suspended for his second Game Misconduct Penalty automatically for the next championship or tournament game of his team.
(c) A Gross Misconduct Penalty involves the suspension of a player or team official for the balance of the game and the offender shall be ordered to the dressing room, but a substitution shall be permitted immediately. Any player or team official incurring a Gross Misconduct Penalty shall be suspended from participating in any further games until his case has been dealt with by the proper authorities.
Note: For Game Misconduct or Gross Misconduct Penalties regardless of when imposed, a total of twenty minutes shall be charged in the records against the player; or against the team in the case of a team official incurring a Game Misconduct or Gross Misconduct Penalty. Such penalties shall be reported by the Referee to the proper authorities immediately following the game. In championship games, league games, or in a tournament any player or team official assessed a Gross Misconduct Penalty shall be suspended for the next game of his team as a minimum.

(a) A "MATCH" Penalty involves the suspension of a player for the balance of the game and the offender shall be ordered to the dressing room immediately. A substitute player is permitted to replace the penalized player after five minutes playing time has elapsed.
Note: For all Match Penalties, regardless when imposed a total of twenty-five minutes shall be charged in the records against the offending player.
(b) A player incurring a Match Penalty shall be suspended from playing in any further games until his case has been dealt with by the proper authorities.
Note 1: The Referee is required to report all Match Penalties and the surrounding circumstances to the proper authorities immediately following the game in which they occur. In championship and tournament games any player who incurs a Match Penalty shall be suspended for the next game of his team as a minimum.
Note 2: If any player is assessed a Match or Gross Misconduct Penalty in any IIHF sanctioned game or tournament, the Organising Committee shall notify his National Association immediately by fax or cable and forward the Referee's Report as quickly as possible.

(a) When there is an infraction of the rules that calls for a Penalty Shot not involving a Major, Misconduct, Game Misconduct or Match Penalty, the non-offending team shall be given the option of accepting the Penalty Shot or having a Minor Penalty assessed to the offending player. If, however, a Major, Misconduct, Game Misconduct, Gross Misconduct or Match Penalty is incurred with the Penalty Shot, the shot shall be awarded and the penalty for the prescribed infraction shall be assessed.
(b) Any infraction of the rules which calls for a Penalty Shot shall be taken as follows:
The Referee shall cause to be announced over the public address system the name of the player selected by the team to take the shot. He shall then place the puck on the centre face-off and the player taking the shot will on the instruction of the Referee play the puck from there and shall attempt to score on the goalkeeper. Once the player taking the shot has possession of the puck he must proceed towards his opponent's goal line, and once the puck is shot, the play shall be considered complete. No goal can be scored by a second shot of any kind, and any time the puck crosses the goal line the shot shall be considered complete. Only the goalkeeper or alternate goalkeeper or a player designated as a goalkeeper or alternate goalkeeper may defend against the Penalty Shot.
(c) The goalkeeper must remain in his crease until the player taking the Penalty Shot has touched the puck, and in the event of violation of this rule or any foul committed by the goalkeeper, the Referee shall allow the shot to be completed, signalling such violation by raising his arm, and if the shot fails, he shall permit the Penalty Shot to be taken over again. If the goalkeeper leaves the goal crease too early
the first time - warning and a new Penalty Shot
the second time - Misconduct Penalty and a new Penalty Shot
the third time - award a goal.
The goalkeeper may attempt to stop the shot in any manner except by throwing his stick or any other object in which case a goal shall be awarded.
Note: See Rule 633, Throwing Stick
(d) In cases where a Penalty Shot has been awarded the Penalty Shot shall be taken by any player selected by the Captain of the non-offending team except a player serving a penalty or a player to be assessed a delayed penalty. Such selection shall be reported to the Referee and can not be changed.
If, at the time a Penalty Shot is awarded, the goalkeeper of the penalized team has been removed from the ice and substituted for by another player, including the substitute goalkeeper, the goalkeeper shall be permitted to return to the ice before the Penalty Shot is taken.
(e) While the Penalty Shot is being taken, players of both sides shall withdraw to the sides of the rink and behind the center red line.
(f) If, while the Penalty Shot is being taken, any player of the opposing team shall have by some action interfered with or distracted the player taking the shot and, because of such action, the shot should have failed, a second attempt shall be permitted and the Referee shall impose a Misconduct Penalty on the player so interfering or distracting .
(g) If a goal is scored from a Penalty Shot, the puck shall be faced at center ice in the usual way. If a goal is not scored, the puck shall be faced at either of the end face-off spots in the zone in which the Penalty Shot has been attempted.
(h) Should a goal be scored from a Penalty Shot, a further penalty to the offending player shall not be assessed unless the offence for which the Penalty Shot was awarded was such as to incur a Major, Misconduct, Game Misconduct, Match or Gross Misconduct Penalty, in which case the penalty prescribed for the particular offence shall be served.
If the offence for which the Penalty Shot was awarded was such as would normally in a Minor Penalty, then regardless of whether the Penalty Shot results in a goal or not, the Minor Penalty shall not be served.
(i) If the foul upon which the Penalty Shot is based occurs during actual playing time, the Penalty Shot shall be awarded and taken immediately in the usual manner, not withstanding any delay occasioned by a slow whistle by the Referee, to permit the play to be completed which delay results in the expire of the regular playing time in any period. The time required for the taking of a Penalty Shot shall not be included in the regular playing time or any overtime.

(a) A goalkeeper shall not be sent to the penalty bench for an offence which results in a Minor, Major or Misconduct Penalty, but instead the penalty shall be served by another member of his own team who was on the ice when the offence was committed, said player to be designated by the Manager or Coach of the offending team through the Captain and such substitute shall not be changed.
(b) Should a goalkeeper incur a second Major Penalty in one game, he shall also receive a Game Misconduct Penalty.
(c) Should a goalkeeper incur a Game Misconduct Penalty or a Gross Misconduct , his place shall be taken by the substitute goalkeeper, if available, otherwise by a member of his team, who shall be permitted 10 minutes to dress in the goalkeeper's full equipment.
(d) Should a goalkeeper incur a Match Penalty, his place shall then be taken by the substitute goalkeeper, if available otherwise by a member of this team, and such player will be allowed the goalkeeper's equipment. However, any additional penalties as specifically called for by the individual rules covering Match Penalties will apply, and the offending team shall be penalized accordingly. Such additional penalty shall be served by another member of the team on the ice at the time the offence was committed said player to be designated by the Manager or Coach of the offending team through the Captain.
(e) A Minor Penalty shall be imposed on a goalkeeper who leaves the immediate vicinity of his crease during an altercation .
Note: All penalties imposed on a goalkeeper, regardless of who serves the penalty shall be charged in the records against the goalkeeper
(f) If a goalkeeper participates in the play in any manner when he is beyond the center red line, he shall be assessed a Minor Penalty.

(a) If a third player of any team is penalized while two players of the same team are serving penalties, the penalty time of the third player shall not commence until the penalty time of one of the two players already penalized shall have elapsed. Nevertheless, the third player penalized shall at once proceed to the penalty bench, but may be replaced on the ice by a substitute, until such time as the penalty time of the penalized player shall commence.
(b) When any team has three or more players serving penalties at the same time and because of the delayed penalty rule, a substitute for the third offender on the ice, none of the three penalized players on the penalty bench may return to the ice until play has been stopped unless by reason of the expiration of penalties, the penalized team is entitled to have more than four players, including the goalkeeper, on the ice in which case the penalized player shall be permitted to return in the order of the expire of their penalties. Otherwise, when play is stopped the player(s) whose penalty has expired may return to the game.
(c) If the penalties of two players of the same team will expire at the same time, the Captain of the team will designate to the Referee which of such players will return to the ice first and the Referee will instruct the Official Scorer accordingly.
(d) When a Major and a Minor Penalty are imposed at the same time on two or more players of the same team, the Official Scorer shall record the Minor as being the first of such penalties.
Note: This applies to the case where the two penalties are imposed on DIFFERENT players of the same team. See also Note 2 to Rule 402, Minor Penalties.

(a) Should an infraction of the rules which would call for a penalty be committed by a player of the side in possession of the puck, the Referee shall immediately blow his whistle and assess the penalty to the offending player. The resulting face-off shall be made at the place where the play was stopped, unless the stoppage occurs in the Attacking Zone of the player penalized, in which case the face-off shall be made at the nearest face-off spot in the Neutral Zone.
(b) Should an infraction of the rules. which would call for a penalty, be committed by a player of team NOT in possession of the puck, the Referee shall signify the calling of a penalty by raising his arm and, upon completion of the play (as defined in Note 2 below) by the team in possession, will immediately blow his whistle and give the penalty to the offending player.
Note 1: The subsequent face-off shall be made at the place where the play was stopped, unless during the period of a delayed whistle due to a foul by a player of the side NOT in possession, the side in possession ices the puck, shoots the puck from is defensive zone so that it goes out of bounds or is unplayable, then the face-off following the stoppage shall take place in the Neutral zone near the Defending Blue Line of the team shooting the puck. If the penalty or penalties to be imposed are Minor Penalties, and a goal is scored on the play by the non offending team the first Minor Penalty shall not be imposed, but all other Minor, Major or Match Penalties shall be imposed in the normal manner
Note 2: "Completion of the play by the team in possession " in this rule means that the puck must have come into the possession and control of or intentionally directed by an opposing player or goalkeeper, or has been "frozen" This does not mean a rebound off the goalkeeper, the goal or the boards, or any accidental contact with the body or equipment of an opposing player
Note 3: If after the Referee has signalled a penalty, but before the whistle has been blown, the puck shall enter the goal of the non offending team as the direct result of the action of a player of that team, the goal shall not be allowed and the penalty signalled shall be imposed in the normal manner.
If when a team is "short-handed" by reason of one or more Minor or Bench Minor Penalties, the Referee signals a further Minor against the team, and a goal is scored by the non-offending team before the whistle is blown, then the goal shall be allowed, and the delayed penalty signalled shall be washed out but penalties being served in the box will remain in effect.
(c) Should the same offending player commit other fouls on the same play, either before or after the Referee has blown his whistle, the offending player shall serve such penalties consecutively.
(d) If any fouls are committed after the play has been stopped, the offending players shall be penalized as though play were actually in progress.

In addition to the suspensions imposed under these rules, the proper disciplinary authority may, at any time after conclusion of the game, at their discretion investigate any incident that occurs in connection with any game, and may assess additional suspensions for any offence committed during the pre-game warm-up, on the way between the dressing room and the ice surface, during the course of a game or any aftermath thereof by a player or team official, whether or not such offence has been penalized by the Referee.
Note: For championship and tournament games, such infractions are to be dealt with before the next game of the team(s). National regulations shall be followed for national championships.
