Definition:
There is no commonly accepted definition but according to US based The Markle Foundation's Connecting for Health collaborative, a public-private endeavor-"An electronic application through which individuals can access, manage and share their health information, and that of others for whom they are authorized, in a private, secure, and confidential environment".
According to AHIMA: The personal health record (PHR) is an electronic, universally available, lifelong resource of health information needed by individuals to make health decisions. Individuals own and manage the information in the PHR, which comes from healthcare providers and the individual. The PHR is maintained in a secure and private environment, with the individual determining rights of access. The PHR is separate from and does not replace the legal record of any provider.
Information stored in a typical personal health record
* Your name, birth date, blood type and emergency contact
* Date of last physical
* Dates and results of tests and screenings
* Major illnesses and surgeries, with dates
* A list of your medicines, dosages and how long you've taken them
* Any allergies
* Any chronic diseases
* Any history of illnesses in your family
Different types of Personal Health Record storage methods:
1.Paper based systems: These are the current methods of health information storage,wherein the patient uses paper or notebook to store his or her along with family medical history.This could be done in consultation with their health care provider.Main drawback of these system of storage are that they are not secure and if lost,data cannot be retrieved.
